  • Download the form, “Consent Letter for Automatic Insurance Premium Payment and Receipt of Benefits via Bank Account” here.
  • Complete the form and enter card information for a VISA or MASTER card only in Clause 1.
  • Send the document by email: pos.document@azay.co.th

  1. Download the form, “Consent Letter for Automatic Insurance Premium Payment and Receipt of Benefits via Bank Account” here.
  2. Complete the form and enter bank information for a savings bank account in Clause 1.
  3. Send the form to the Policy Management Department on the 1st Floor of Phloen Chit Tower Building, No. 898, Phloen Chit Road, Pathum Wan District, Bangkok 10330.
